This may be useful either when initiating long-term therapy for a patient with uncontrolled asthma or as a short `rescue' course at any stage for acute exacerbation. Long-term oral corticosteroid therapy may be required to control severe persistent asthma, but its use is limited by the risk of significant adverse effects. In these cases high-dose inhaled corticosteroids should be continued so that oral requirements are reduced to a minimum. Oral doses should be given as a single dose in the morning to reduce the disturbance to the circadian cortisol secretion. Dosage should always be adjusted to the lowest dose which controls symptoms. DCR is generally deferred below 18 months of age. Methods: 30 cases of paediatric dacryocystitis in the age group of 8 months to 15 yrs were randomly divided into 2 equal groups. Group I Paediatric DCR with bicanalicular silicone intubation BCSI ; n 15 ; & group II Paediatric DCR without silicone intubation. DCRs were performed through a straight incision 4 5mm medial to medial canthus, 12 x 15mm osteotomy & single anterior flaps of sac & nasal mucosa. BCSI was performed in group I. Silicone stent was removed after 3 to 6 months. Results: 100 % success rate was achieved in both the groups during 3 months to 15 months followup. However stent related complications were noted in group I, i.e., time consuming & complex surgery, prolapse of stent at ocular end, cheese wiring of canaliculus, extrusion, dirty nose, pain & irritation. Conclusion: BCSI increased the complexity of paediatric DCR without increasing its success rate & caused additional operative & postoperative complications. Plane DCR is equally successful. As they evaluated age-matched control subjects and investigated patients using both conventional and tissue-Doppler echocardiography, which is a modern and accurate method, their findings of alteration in RV diastolic function may be considered to be robust. As discussed in their article, 1 RV involvement may have a different origin, including primary myocardial involvement, lung fibrosis, and or pulmonary artery hypertension. Based on the absence of LV or systolic abnormalities and extensive lung fibrosis, they assumed that RV diastolic abnormalities are consistent solely with pulmonary artery hypertension; they also addressed a few limitations concerning the assessment of pulmonary artery pressure by resting echocardiography. However, to our knowledge, the absence of systolic abnormalities using conventional techniques does not rule out primary myocardial involvement.2 Moreover, diastolic function may be altered very early in the course of SSc, and may be consistent with primary myocardial involvement.3 Even more intriguing may be the absence of LV systolic, LV diastolic, and RV systolic abnormalities. In fact, we and others have previously highlighted that patients with SSc often exhibit myocardium perfusion abnormalities in up to 100%2 together with LV diastolic dysfunction, 3, 4 and also RV systolic dysfunction3; moreover, these alterations were demonstrated in both cutaneous subtypes of the disease and in patients with normal pulmonary artery pressure and early manifestations of SSc duration of disease, 5 years ; .3 The question is how to explain the absence of any alterations in both LV.
